    1. The above-named Guest does hereby consent to the recording and distribution of reproduction(s) of the Guest's voice and or image and performance as part of the media program (audio and video podcast) titled “Choices & Chances” (herein referred to as the "Program").

    2. This is to confirm that the undersigned has agreed to be interviewed or otherwise participate in the Program.
     
    3. As a condition of publication/broadcast, this for no monetary compensation unless otherwise agreed in writing, requests the non-exclusive worldwide rights to reproduce, distribute, and sell your oral and/or video presentation, in whole or in part, in any media, as part of this program published under the auspices of and to license these rights to others, in perpetuity, shall have the right to edit and/or transcribe your presentation.

    ​4. I understand that the Program at its sole discretion, produce presentations or publications based, in whole or in part, upon audio and video interview (or any portions thereof) audio/video recordings or photographs/stills of said interview and that such media or transcripts may appear in print, online, or in any manner of/or media, including but not limited to promoting the podcast or streaming audio and video program.
     
    5. You also grant the right to use your name, biography and likeness in connection with the Program.
     
    6. You warrant that your presentation is original with you, that publication/broadcast will not infringe on the rights of others, and that you have full power to grant this license. Should your presentation/broadcast incorporate copyrighted materials of others, you warrant that you have obtained such permission from those parties for publication/broadcast by , consistent with this Agreement.

    7. This grant of permission does not include the release of any of the Guest’s confidential information. For purposes of the paragraph, ''confidential information'' is defined as your home address, email address, telephone number or date of birth of the guest, unless the guest gives us their consent.

    8.1 that you the guest will not discuss any matters before any courts in Australia, Commonwealth, State, or Territory or identify anyone before any courts in line with Australian law on any audio or video recordings, any live streaming event or in any written communication.

    8.2 That you the guest will not defame, harass, slender, say anything that is not truthful, or use any statistics or information that has not been thoroughly sourced about any person or group—business or Government agency.

    8.3 That the guest recordings are in line with community expectations. The Program has the right to refuse to record or release any audio, visual, or written works, that are illegal or immoral or go against the Program's code of conduct and will delete such recordings.

    9. I hereby consent to the use of my oral and/or video presentation as described and agree with the provisions of this release form and will abide by the terms and conditions.

    10. If the foregoing terms are satisfactory, please sign and date this agreement below and return it to the program coordinator. Execution of this Agreement does not obligate to publish your presentation or other materials.

    This release shall be binding upon me and my heirs, legal representatives, and assigns.
